Support Coordination Services
When you want to take control and build capacity towards a more independent life, our team of Support Coordinators are here to help. Setting you on a path towards your personal goals, we will work with you to discover your options. Our team can assist you in finding the right services, build better networks of support, and adapt your services when your needs or goals change.
Levels of Support Coordination
Support Coordination is available at 3 levels, support connection, coordination of supports and specialist support coordination, with each level depending on the requirements outlined in your NDIS plan and the length of time needed to facilitate your support. This service offers reassurance, guidance and ongoing support whilst navigating the NDIS. There are three levels of support coordination that can be included in your plan:
Level 1
Support ConnectionSupport Connection
This level of support builds your ability to connect with informal, community and funded supports enabling you to get the most out of your NDIS plan and pursue your personal goals.
Level 2
Support CoordinationSupport Coordination
A common support level that helps you to better understand, implement & use your plan. A Support Coordinator will work with you to ensure a mix of supports are used to help you live more independently.
Level 3
Specialist Support CoordinationSpecialist Support Coordination
A higher level of support coordination. This service is for people whose situations are more complex and require specialist support.

We have a team of dedicated Support Coordinators that will help you understand and implement the funded supports in your plan and link you to the community, mainstream and other government services. We focus on supporting you to build skills, connect with providers and assist you in optimising your NDIS plan.
- Understanding your NDIS plan
- Identify the supports that are available to you
- Determining monthly and weekly budgets aligned to plan categories
- Maximise your funding by obtaining quotes for services, equipment and home modifications
- Coordinating service agreements and service delivery with Service Providers
- Negotiating and liaising with Government, NDIA and other agencies
- Monitoring supports to ensure you maintain a clear pathway to achieving your plan goals
- Resolving any issues that may arise with Service Providers
- Requesting early plan review or to submit a change of circumstances to the NDIA
